As set forth on our home page, The Michael S. Jacobs Foundation’s main goal is to award four $2,000.00 college scholarships to high school seniors. The scholarships are based on academic performance, community service, and participation in either varsity baseball or golf. We are able to award these scholarships through private donations as well as holding a yearly golf tournament.Visitors accessing our Website, which can include children under the age of 13, can access all features of the site. Registration forms are included on the Website in order to sign up for the available scholarships, make donations and sign-up for tournaments.

The types of personal information we may collect at this Website from children under age 13 include: first and last name; home address; e-mail address; phone number; fax number; age/birth date; emergency contact and emergency phone number; and shirt sizes.

We do not condition (and are prohibited by law from conditioning) a child’s participation on the child’s disclosing more personal information than what is reasonably necessary to participate in an activity held by the Michael S.
